Any injuries/adverse events to subjects involving risk are reported
      to the IRB within one working day of occurrence.
Researchers submit annual reports to the IRB for all Expedited
      and Full Board studies.
Modifications to approved studies are not implemented before
      they receive written approval without contingencies.
